I was there last week and got the extension. I was not asked for the letter, and on the form for the extension there is a box you can tick that says you are unable to get it from your embassy
I was not asked but I did have a copy of my tm30 I gave with all my passport copies so I think it may be needed as the lady at the copy place around back seems familiar with the pages to copy
Most immigration offices are aware of which embassy would issue those letters. What happened to someone from Country A may or may not is going to be applied to the person from Country B at the same immigration office.
You might be better off asking in a Chiang Mai related group, but I know we have a few members from Chiang Mai in here as well who might know the answer. Remember though, what happens to one person may not apply to you.
