I was not asked but I did have a copy of my tm30 I gave with all my passport copies so I think it may be needed as the lady at the copy place around back seems familiar with the pages to copy
I was there last week and got the extension. I was not asked for the letter, and on the form for the extension there is a box you can tick that says you are unable to get it from your embassy
