That medical cert is but a CYA type doc most clinics receptionist sells you for 100-200 THB. You only need ID and be still breathing.

The doctors surgery closest to the DLT centre is usually one that is set up churn that form out cheaply and quickly in most towns.

You can just go to any doctor/clinic by the road and they will know. Paid 50 baht one time in Buriram and didn’t even see the doctor. In the tourist spots I’ve seen people pays around 200 baht for this simple statement.
