TLDR : Answer Summary
To obtain a medical certificate for a Thailand visa in the UK, download the medical certificate form from the Thai embassy's website, specifically for the Non-Immigrant Type O-A visa. After downloading, take the form to a doctor in the UK for completion and signature. Make sure the certificate is issued no more than three months prior to your visa application.

You take it to a doctor in the UK and you get it signed.
