Hello. I have some questions about the visa application form as it confuses me:
1. In the top right I have to select which visa I want, but “DTV” is not one of the options, so which one should I select?
2. “Current address”: I’ve been renting a Bangkok apartment for over a year, but I’m going to Jakarta to apply for my DTV, so which address should I write down? My Bangkok condo or my Jakarta hotel?
3. “Permanent address”: I guess I should I leave it empty as it will be the same my Bangkok address above? Or perhaps I should even note my Netherlands address where I grew up?
4. “Date of Arrival in Thailand”: Ehh I don’t know the exact date upon which I’ll enter Thailand again, as it depends on when they’ll approve my visa lol. So what should I fill out there?
Thanks for any help :) !

TLDR : Answer Summary
A user is seeking clarification on completing a visa application form for a DTV (digital nomad visa) while applying from Jakarta. Key issues include the appropriate selection for visa type, which address to use, and how to handle the date of arrival given uncertainty about entry date. Responses from the community suggest various approaches such as writing 'DTV' as an additional option, using the current address in Jakarta, listing a home country address for the permanent address, and estimating the arrival date.
Here’s what I did, and they accepted… 1. write DTV. 2. The hotel in Jakarta 3. I put my “home country” address’. 4. Whatever date. I put 10 days after I submitted application
Current address I put the place I'm staying in Jakarta 'currently'
Permanent address in home country and proposed address in Thailand is obviously my Thai address. I'm almost at the front of the queue so I'll update you in a moment, I hope
the embassy in Manila told us to write DTV in the box EDIT: I meant in the top right corner next to the other options. On our form it had a box around all of the other visa types.
