What should I fill out for my visa application when applying through Taipei for Thailand?

This is a question for anyone who has applied through Taipei.

When filling out the application form for visa, I am unsure on what to write in certain boxes so would love some help from someone who has been successful in the process.

I.D No: (I thought this might be passport number but they ask for that too, so unsure what this is)

Purpose of visit to Thailand: (I was thinking about ticking “other” and writing DTV?)

Proposed duration of stay in Thailand: (6 months? 5 years? Again, unsure what is best to say)

And finally: Place of stay in Thailand: (do I just write the address of a random hotel? Because it certainly won’t be the place I’ll be staying for long, but maybe for the first week)
