In the red box is asks for:
-proposed address in Thailand
-name and address of local guarantor
I currently don’t have either a proposed address in Thailand or a local guarantor. Did people just leave this part blank or how did you navigate it?
Please also add what location you did your application as it helps others searching for answers

When filling out the visa application form for Thailand, if you do not have a proposed address in Thailand or a local guarantor, you can enter the address of a hotel where you have made a booking. It is generally acceptable to leave the local guarantor section blank. It's advised to note 'N/A' for the guarantor and to indicate DTV as the visa type on the top right of the form. If there are questions about the duration of stay, clarify whether to write 180 days or 5 years for your proposed stay.
It's just a generic template visa application, as you can see it doesn't even list DTV. Just enter a hotel name and put N/A for sponsor/guarantor. Fill in DTV for the visa type top right
Please fill in the hotel address you booked in Thailand or any other address, this type of visa does not require a sponsor, you can leave that part blank.
