How do I fill out my DTV visa application while applying in Jakarta?

Looking for clarification on how to fill out my application for the DTV visa, applying in Jakarta:

For “current address” - is that my hotel in Jakarta, the house I rented in Thailand (rented until next year), or or my permanent address back home in USA? — if I should put my Thai address, then I assume the next question, “Permanent address, if different from above” should be answered with my permanent address in USA?

For “date of previous visit to Thailand: would I answer with my last entry date or exit date?

I have a USA passport - How should I answer “countries for which travel document is valid”?

And finally, for “Name and Address of Local Guarantor” and “Name and Address of Local Guarantor in Thailand” — do these need to be filled in?

We filled the form in Jakarta and this is how we did it

Current address is where you are rn

Permanent address is US in your case.

Date of previous visit. Put the date you enter and exited last time

Countries for which travel document is valid: All countries

We did not fill in the guarantor as the DTV does not require one.

The form is general to all type of visa and not DTV specific.

Date of previous visit: Only allows for one date or can you supply entry and exit dates? Can’t remember what I wrote (I wrote it standing outside the Phnom Penh embassy, using a tree as a base prop up my form), but maybe include entry and exit. Perm address is always the US. I filled in the guarantor in Thailand because there was a random person (maybe associated with the embassy) telling us we need to answer every question. I entered my Thai partner’s info, but then was worried, while waiting for approval, since I didn’t supply any type of guarantor letter.

Only temporarily lifted at a small number of checkpoints. I've never not had to fill one out when entering by land and I've been through MOST land border checkpoints.

The TM6 IS still required for land entry. I filled one out less than 2 weeks ago. Someone who doesn't know what they're talking about shouldn't claim to know such things.

Permanent address can be in Thailand. Many people don't have an address in their home country. I don't think that part is very important. Put something down, they won't really care what it is.

don’t know. You are applying for a 180 day glorified tourist visa, thst is good for 5 years. To me, it would be strange to put down your “permanent address” as Thailand.

Although some people don't have a residence back home. On the TM6 forms still handed out at land borders, I always put my Thai address down as my "permanent address". While I own investment properties abroad and I could always put down a family member's address, I'm being honest when I write my address in Thailand on that form and have never been questioned about it (and believe me, I travel a lot). In fact, since the borders re-opened, I've crossed land borders into Thailand probably at least 15 or 20 times and that doesn't include the numerous trips to Europe/Australia/Vietnam/Singapore and other countries I've made by air in that time.

On a visa application, I generally do the same thing (unless it's an actual tourist visa). I guess it depends on the circumstances and if a visa officer actually requested an overseas address be put down, that's when you do (or ask them for clarification should you not have one).

- TM6 isn’t required by air for several years and has not been required by land entry for six months now. Someone who travels a lot should probably know that. Greetings from Jakarta.
