TLDR : Answer Summary
If you've misplaced your 90-day report, the first step is to determine how strict your local immigration office is. Experiences vary, with some offices allowing continued applications without the report, while others may require a police report stating that you've lost it. If you filed your last report online, you can simply print another copy. Having your passport and showing up on time is crucial. It's generally noted that your data is in their system, so the physical paper is secondary; however, having a police report can be advantageous in case of police checks.

The last time I filed it in person and I'd lost the paper. The IO didn't say anything. Just told me to make sure I didn't lose it again the next time. It's now stapled in my passport.

No problem at all. Take passport and make sure you go on time. Do not say anything. You data is in the computer. The paper is secondary. Might be an issue if you get stopped by the police though. But Tod Daniel's is right. Maybe a notice from the police might be better, just in case.

If you did the last one online, just print off another copy of the report

totally depends on how hard core the immigration office you deal with wants to be.
I have seen them do nothing and let people apply for their next 90 day report just fine AND I've seen them turn people away, make them go get a police report stating you lost the previous receipt before you can file again.
I advise people to go get a police report (just in case you do need it you have it) before you go to do your next 90 day report.
