TLDR : Answer Summary
If you lose your 90-day report slip in Thailand, it's recommended to first try and retrieve it through the system if you submitted your last report online, as you can access your previous reports. If that's not possible, visiting a police station to file a report about the lost slip is advised. You can then go to the immigration office and file a new report. Fortunately, you have a 21-day window (up to 2 weeks early and 1 week late) to file without incurring any penalties.

If you did the report online and got the approval that way you can go back into the system and retrieve the data of ANY previous 90 day report you did online that was approved. Otherwise, get the police report and go file around when you think it's due..

I recommend you find it π Otherwise, go to a police station get a police report stating you lost it and then go when you think it's due to the immigration office you use and file a new 90 day report. The good news is you can apply up to 2 weeks before the due date or up to a week after the due date without any penalty SO you have a 21 day window of opportunity to get the report done without a fine. π
Good luck, I say get a police report stating you lost it because some immigration offices are hard core about you doing that. Believe me you're not the first foreigner who lost that 90 day report receipt
