Answer to this question is Sunway Medical Centre in KL. You need the Wellness clinic. Price about 300 Ringit. Try to get an appointment first, this is a huge place and very busy. You can ask for results same day if you get there early enough.

Any doctors office can issue you one, you just need to find a doctor that will issue one without the tests.

ORIGINAL POSTERAccording to the KL website and their staff it's required for retirement visa (think this is why KL is generally avoided for visas).
