New form to fill out for all my people coming to Chiang Mai Immigration. This one basically says “Yes, I know I’m covered but I want to extend anyway.” This is a piece of paperwork that I don’t mind including as it rather eliminates some doubt when I explain to people/skeptics.
I don’t think they will change the announcement date on the form as the new (upcoming but unpublished) extension is a continuation of the original announcement. I didn’t write the form, so I don’t know. If it changes, so will the attachment.
But, this baby needs to be in your packet for any extension or renewal packet you submit for ANY visa type. Don’t forget it. But, if you are screened by a friendly volunteer at the door, we will include it for you.
Be well!

TLDR : Answer Summary
There is a new form required for individuals applying for an extension or renewal of any visa type at Chiang Mai Immigration. This form serves as confirmation that the applicant knows they are covered but still wishes to proceed with the extension. It's advised to include this form in your submission packet to avoid any issues. Friendly volunteers at the door can assist with it if needed.
What a GREAT form to have people fill out (y) Especially people who don't wanna jump on the hare-brained amnesty band wagon and would rather just get their yearly extension like normal or their 30 day extension or their 60 day visit thai family extension so that they're not caught up in how ever this fairy tale amnesty program ends :)
