A re-entry permit is valid until the end of the stamp you bought it with. So when your entry stamp is done so is your re-entry permit. You would need to be back in Thailand to apply for an extension before that ends. A re-entry permit does not give you any extra time in Thailand and your stay continues ticking down when you're outside Thailand.
yes you can use it until that date. You just write a date on the form. It doesn't matter. It's valid from when they put it in your passport until your expiration date.
