Affidavit of a Family Member Form

Immigration gave me this form for my visa extension. Support of a Thai child

Its a new form to me, but I want to get it right. As I understand it it's a family member confirming your family.

The first part looks like its filled in by the family member, can this be anyone? Wife? Mother in law?

Second part my details?

Third part

Signature 1. Family member

Signature 2. Competent officer? Imm officer?

Signature 3. Witness? Who can sign?

TLDR : Answer Summary
The Affidavit of a Family Member Form is required for visa extensions involving support from a Thai child. The form needs to be filled out in three parts: the first by the family member (which can be anyone like a wife or mother-in-law), the second with your details, and the third includes signatures—one from the family member, one from the immigration officer, and a possible witness. Witness requirements may include providing a house book and ID copy. Form STM.10 must be filled out by the Thai citizen child or by the foreign parent if the child is underage, and this form is mandatory at Thai Immigration offices.

If you arrive on a 60 day exempt then get a 30 day extension. Can you use this form to get another extension if you have a Thai child? Thanks.

@Jamie *******
no you use the TM7 application for extension form

BUT

we're seeing more and more offices not allow the 60 day visit thai family extension on free stamp/tourist visa entries so check with your office well before you're going to attempt it to see if it can even be done there

You normally require a copy of the house book and ID card of your witness, signed and dated.

@Ron *****
and you are speaking from experience, having used this form at the immigration office where you get your yearly extension?

I've used this form, we used a neighbour that knows us. We were told by our immigration office that a friend or headman was acceptable as well. Not sure about a relative, that's why I said check with his local office.

@Ron *****
thanks for that. Appreciate the info..

Unless it has recently changed, the STM.10 was not on the list of forms for download from the official website. It was handed out by immigration offices when they required it to be completed. So if they give you one best to check who can witness as like everything requirements are office specific.

It's listed here

Neighbour, friend, village headman if you live rural. Relatives are usually accepted but maybe check with your immigration office. Basically anyone who knows you.

Form STM.10 is filled out by the Thai Citizen Child (or in the viewpoint of the Thai Citizen Child if the Thai Child is not yet of legal age)

if the Thai Child is not yet of legal age, then you as the Foreigner Parent would sign Form STM. 10 on behalf of the Thai Citizen Child, or the Thai Immigration Officer will give instructions for what to do

in Sections 1, 2, and 3, the Name/Details to specify are you as the Foreigner Parent

this Form STM. 10 is mandatory and used by all Thai Immigration Offices across Thailand

your data in the middle, family member on the top, immigration officer signs it, never seen that anyone had to witness it before

they started handing those out in Bangkok this last year for marriage/raising child extensions
