What are the immigration forms given for a marriage extension in Thailand?

seems to be like a witness statement, but it asks things like where and when we met and did we marry for money, as well as about family. Its the sort of thing you'd expect them to ask when we're there, but not on a form - seems like someone else should be asking the questions, and my wife thinks it should be for our witnesses to complete.
