Can anyone explain what these forms are we were given by Korat Immigration? My wife isn't sure. They're for my year's extension based on marriage to a Thai. Thanks.

TLDR : Answer Summary
The forms provided by Korat Immigration are likely related to the annual visa extension based on marriage to a Thai citizen. Comments indicate that these forms may involve documentation or testimony from the Royal Thai Police regarding the marriage and hometown.

seems to be like a witness statement, but it asks things like where and when we met and did we marry for money, as well as about family. Its the sort of thing you'd expect them to ask when we're there, but not on a form - seems like someone else should be asking the questions, and my wife thinks it should be for our witnesses to complete.
