Has anyone filled out the page two of the TM8 form lately, (re-entry permit). I’m a bit confused, especially with what to put on the top line. I assume by the previous visa they’re asking for information details of you’re last extension of stay. I will renew my extension of stay one day this week, so any help would be greatly appreciated. I will be renewing at Bangkok immigration.

TLDR : Answer Summary
A user seeks clarification on filling out page two of the TM8 form for a re-entry permit in Thailand, specifically what to enter for the previous visa details as they are renewing their extension of stay. Several commenters confirm the correct usage of the form, advise that the re-entry permit is valid only until the current extension expires, and suggest getting the re-entry permit on the same day as the extension renewal.

That’s the correct form for a extension, the forms are available at the small kiosk near DTAC at Changwattana building B from 7am at cost of 3b.

Must be a new version of the form. I always have used the one page version from their website and they never refuse it. Do your extension first. Then do the reentry permit.

The reentry permit will only be valid until your current extension of stay ends. So get the reentry permit the same day after you get your extension of stay to get a full year if you want a multiple reentry.

Your re-entry permit is only good until your current extension expires. So you need to get your extension first then your re-entry. The date you will use would be the date that your new extension of stay expires.

Where did you get that form? Does not look like TM8 I have ever used.
