French citizen with a 30 days visa on arrival. 

What papers do I need to get the 30 days extension at the immigration office in Phuket?

There is no 30 day visa on arrival for people from any country :/ That means you have a 30 day visa exempt entry ;)

The required documentation is your passport copies, passport photo and a filled out application
