Apart from passport and photos required in order to extend the visa on arrival for 30 days, is proof of new air ticket, insurance extension, and accommodation also required? Can you apply three weeks before expiry?

Thanks all. I have extended from the exit date for further 30 days. Just to let you know ....On the phone they told me to go to office near Silom. I went there and they told me to go to Bldg B, ChangWatana. When I got in there ..they told me to go to IT Laksi. This was just a small office (only one staff doing extension) on 3rd floor of a half abandoned shopping mall. Totally unorganised. But okay in the end.
You don’t need to show a ticket out of Thailand or insurance and you don’t need paid accommodation for the extension
Most offices will let you extend upto 30 days before your stamp expires and it’s added on to the end of your current entry stamp regardless of which day you go
