Depends on your Immigration Office.

If you do not have an address you need to register first where you stay.

If you have an address, only passport and application form TM47 needed.

In some offices only passport needed, they use their automatic system.

BTW In Thailand is the Emergency Decree still in force, that means that you always need to carry your passport with you. This just as extra advice.
