TLDR : Answer Summary
When completing your first 90-day report in Thailand, make sure to bring your passport, the last 90-day report slip, and any relevant forms like the TM28 for your address change. If you're moving to a new address, you'll need to also present a copy of your rental contract, the landlord's blue book and ID, and your passport's information page along with your last visa or extension and entry stamp. It’s essential to fill in the TM47 form during this process.

Moving address first. Assuming you are renting then copy of the rental contract, copy of landlords blue book and ID and copy of your passport information page, you last visa or extension and your last entry stamp. You will also need to fill in a TM28 form. You will be left with a slip in your passport with your new address on it.

With regards to the 90 day report. You need to show your passport and last 90 day report slip (they don't normally need copies). Fill in the TM47 form.
