TLDR : Answer Summary
For the 90-day reporting in Pattaya, apart from your passport, you need to present a form that you fill out upon your first report, and if you are returning for a repeat visit, you should have the small slip named Notification of Stay from your last report or address registration. At the Jomtien Office, they will staple the blue Notification of Stay into your passport.
