What documents are needed for the 90-day report in Thailand, and how early can I submit it?

I’m sure this has been asked, but what documents do I need to take in for my 90 day report? I don’t know the name of it, but I’m sure there is an application to fill out? And also, I think a TM30? (proof of residence where you’re staying?)

What else is needed and how much the way do I have as far as days ahead or after my 90th day to do the report? I prefer to go a few days early in case there were issues.

And if anybody has done this in Phuket, please advise on where you went? Do you just google your local immigration office?

Thank you in advance to all 🙌

TLDR : Answer Summary
To complete your 90-day report in Thailand, you will need the TM47 form and the TM30 form (proof of residence). You can file for the report 14 days before or 7 days after your 90-day mark. The TM47 form is available at the immigration office, which should take about 2 minutes to fill out. In Phuket, you can find your local immigration office by searching online.
