TLDR : Answer Summary
For your 90-day report in Thailand, you need to bring your passport and ensure that your TM30 registration is up to date. A TM47 form may be required by some immigration offices, but it is primarily a report of your address. There is no need to submit bank statements unless specifically requested by the immigration office.

Use the trusted Thailand 90 Day Reporting Serviceto get your in-person report done and mailed to you for as low as 375 THB (even if the online system doesn't work for you).

Unless you were asked by the immigration office to show funds at the 90 day mark when you received your extension of stay you only need your passport and TM30 for a 90 day report, some offices process the report just with your passport.
You don’t need to submit bank statements for your 90 day report, just take your passport, a completed TM 47 form( not all offices require this) plus make sure your TM 30 is up to date.
