TLDR : Answer Summary
To extend a 60-day tourist visa in Chiang Mai, you need several documents: your passport (with copies of the data page, latest visa or e-visa, most recent stamp, and departure card), a TM30 form filed at immigration, specific application forms available at the office, 2 passport photos, and a fee of 1,900 THB. You do not need to obtain an additional 30 days of Covid insurance.

Copy ID page passport

Copy visa page passport EDIT or copy E-Visa

Copy Admitted until stamp in passport

Copy TM6 (white card you filled in on arrival)

Copy address registration

2 passport pictures

Fee 1.900 Thb

Application and 2 other forms you get at the Immigration Office.

Curious how you managed to get a new British passport in less then 2 week.

Great question.
@Kelvin ****
, if you got an emergency passport, there is a chance you may not be able to get an extension of stay on the emergency passport. Talk to the immigration office as soon as possible.

You will be applying for a 30 day extension of stay based on tourism. You will need the following documents:

1. Your passport and:

a. a copy of your data ("picture") page

b. a copy of your most recent visa OR the e-visa (if you have one)

c. a copy of your most recent stamp

d. a copy of your departure card

2. A TM30, filed in the immigration office.

3. The forms (each immigration office may have different forms, and want to use their special forms)

4. 2x passport pictures

5. 1,900 THB.

No you do not.
