TLDR : Answer Summary
To extend your 60-day tourist visa in Chiang Mai for an additional 30 days, you will need to bring several documents to the immigration office at Promenada Mall. These documents include: an application form, one passport-sized photo (note that in Chiang Mai, a larger 4x6 cm photo is required), copies of the ID page of your passport, the current visa page, the 'Admitted to Stay' stamp, and your departure card. If you're staying in a hotel, include a receipt; if you're living in a private residence, provide the small notification paper you received when registering your address. The fee for the extension is 1,900 Baht.

Important with Chiang Mai is they want photos in 4x6cm which is larger than a standard passport photo. You can get them out there for 200B if you don't have them, but cheaper if you can do yourself in advance.

Thank you so much

That's probably close enough for them. They take issue with 3.5 x 4.5 which is the standard EU passport photo size. Just Chiang Mai, I have used 3.5 x 4.5 for visas and extensions everywhere else.

I’ve always given them 5CM X 5CM photos and never a problem. Many times..

For each of you: Application form, 1 passport picture, copy id page passport, copy visa page passport, copy Admitted to Stay stamp passport, copy departure card. 1.900 Thb fee. If you stay in hotel a receipt from hotel, if you stay private house the small paper you got when you notified your address.
