Sure. You can provide your medical records. Just in case I believe you better ask your treating doctor to write medical certificate that you have been receiving treatment with him/her in that specific hospital.
For me dentist will have 2 appointments. This is also from UK - I would email your thai embassy for your country. Another guy from uk only has only one appointment.
