I submitted the application, photo, copy of passport photo page, bank statements for last 6 months, letter from employer with job title, salary, etc. and my rental agreement in Phnom Penh, plus $400.

thank you. Is your employer in Cambodia? If not should the employment letter be notarised/ certified etc? How about the bank statements? Also were did you print the form for the application?

- Employer is USA. Just printed it off. Same with bank statements. Nothing notarized/certified. I got the application at the embassy, as I thought it would be updated(include DTV category), but it was the same application on their website, that is several years old. I just wrote DTV below the other type of visa options

Thank you so much for your helpful advice. As I would not have rental agreement (I'll be in a tourist visa) could I use my hotel reservation in lieu of a lease agreement and also a copy of my passport stamp as a current proof of location?
