What documents are needed for the first 90-day report at Thai immigration?

For the first 90 day report I know I should go to immigration rather than attempt it online. I need my passport, photocopy of info page, photocopy of visa and tm 47 form. Anything else? Thanks all

TM30 paper copy in some offices. Some others just take it from the system.
