What documents and fees are required for notifying immigration in Phuket Town about a change of address?

Good evening Tod and the other wise men. I will be notifying immigration (Phuket Town) as to the change of my address tomorrow. I have my new tenancy contract, copy of the owner's ID and a copy of his house ownership details.

My question is, do I need anything else and is there a fee? Many thanks

TM28 I can get at the immigration office correct? So everything else I have is sufficient, but may need to fill both the 28 and the 30?
