TLDR : Answer Summary
To obtain a medical visa in Thailand when currently on a 45-day visa exempt, you can't actually get a specific 'medical visa'. Instead, look into obtaining an extension of stay on medical grounds. For this, you will need documentation from the hospital where the surgery is scheduled, showing that the treatment (in this case, ACL surgery) is necessary. Extensions may be granted for 30 to 90 days, depending on the immigration office's discretion and the physician's recommendations. The process is generally facilitated by private international hospitals that have staff to assist with the extension process.

If yer son is having surgery at 1 of the big Private International hospitals, they have staff that will help facilitate the extension process.

There is no "medical VISA" you can get inside of thailand ๐Ÿ˜•

You can get an extension of stay based on medical reasons from the immigration office in the area where you stay WITH documentation from the hospital and the treating physician.

Keep in mind they are pretty hard on handing them out and a torn ACL might or might not be reason enough to get one. All you can do is get documentation from the doctor and the hospital showing a surgery is scheduled etc, and go out and try for the extension.

Extensions based on medical need can be issued from 30 to 90 days at the discretion of the immigration office and the recommendations of the hospital/physician, cost is the same as all extensions inside the country 1900baht
