What are the requirements and process for extending a Thai visa at the hospital?

As you can see i got my extension at the hospital.

Now what i was told on the phone was.

Photo copy of your data page.

Photo copy of your visa be it a visa exempt or a SETV.

Photo copy of your Tm6.

One photo of yourself taken no longer than 3 months ago.

You drop that of monday to Friday between 8 in the morning and 5 in the evening.

You don't fill out a tm7 they do that for you.

You go back Saturday between 8 and 12 mid day for you 30 days extension.

For 2800bt.

But if you walk in on a Saturday you pay the usual 1900bt + 1500bt.
