What documents are needed for a 30-day short-term extension at MTT in Thailand?

I'm guessing this is the amnesty extension:

You will need:

1. Your passport.

2. Copies of: (sign each page)

a. Data ("picture") page

b. A copy of the page with visa exempt stamp

c. A copy of the departure card

d. A copy of embassy letter

e. A copy of your entry stamp

3. ONE passport photo (only handed one photo. Others have said 2 is needed. )

4. 1,900 THB fee (be sure to hand it separate from you stack of papers or they will drop it.)

5. A form acknowledging the overstay penalty. They will provide this.

6. TM 7

7. Proof of an appointment booking online. Vital.
