Hello All,

We have to go to Chiang Mai Immigration to extend our 30 day exempt entry stamps from our Mai Sai border run. When we got married the district office gave us documents to present to the immigration office. When I fill out my TM.7 should I use my maiden name or my married name? My (U.S.) passport is still in my maiden name.

Thank you in advance ❣️❣️❣️

TLDR : Answer Summary
When applying for a visa extension using the TM.7 form, it is advisable to use the name as it appears in your current passport. In cases where your passport is in your maiden name, you should fill out the application with that name.
