Heading to CW tomorrow to extend this visa if I can?

I am US, did border run to get this.

Can I get another 30 days or 60 days?

If so what form do I use?

You can get a 30 day extension ONCE on that 30 day visa exempt entry stamp for 1900baht.

Go downstairs to the copy shops, they'll make the copies you need, fill out the form you need and even take your passport sized photo if you don't have one.
