TLDR : Answer Summary
The inquiry concerns the timing for applying for an extension on a Non-O visa based on marriage in Thailand, as well as whether health insurance is necessary for applicants over 50. Community members confirmed no health insurance is required. As for the application timing, it is indicated that one can apply within 30 days prior to the visa expiration date. The poster successfully extended their visa with a 30-day provisional extension while awaiting further processing.

here is what happened today :

No Health Insurance needed ..

I was there at 8:40 , got number and met the officer around 10:30.

I had all docs and copies as requested , everything looked OK. but somehow she (officer) wanted to waste the time! checked the docs , wait!, asked some questions, wait! after lunch asked us in again, send me to copy some docs again (they needed 2 examples!), wait again!, needed extra form to be filled, wait!! time goes pass and go in and out again and again until 16:30 , they gave me back the Pass with 30 days extension (until 06/Feb) , asked me to come back on 03/Feb to check the result av investigation , OK /NOK ! I have to update the bank-account balance at 03/feb and leave new copies again!

In the end everything was as I expected except long time at office !

And it is normal than you file 2 complete sets in case of application on basis of marriage with a Thai National.

Sorry, but I read I had all docs and copies and then read the officer wanted to waste the time and send me to copy docs again. You did not prepare all docs, then extra forms to be filled, you did not prepare these extra forms, you got back your passport with 30 days Extension. No, you got a stamp with the text:

Application of Stay is under consideration of the Immigration Bureau. Applicant must contact the office again on 03 Feb 2020. Not understand which documents you need to copy again if you return that date. Example of the stamp

he has a Visa already and wants to apply for an extension of stay out of this visa. He can do this within the last 30 days of the validity of his visa. The 15 days you mentioned are the limit for making a "conversion" to a Non-Imm-O visa from either a tourist visa or an exempt visa

if he has all required documents at hand and ready, 400,000 THB in his bank account for at least two months plus the banks "letter of guarantee", or meets the income requirement of 40,000 THB per month, documented by a notarized letter by his embassy, he can apply for the yearly "extension of stay based on marriage to a Thai" at Chaeng Wattana anytime now within the last 30 days of his Non-Imm-O visa based on marriage. No health insurance is required for this

you will most probably get the new stamp for a 1-year Extension of Stay on the same day you apply, given your documents are a duck in a row. arrive early at CW and pull your number

I was told at CW in BK while applying for NoN O based on marriage that I needed a minimum of 15 days left on my Visa to apply. I would assume you will need to get a visa extension beforehand.
