My 6 month multiple visit visa runs out in October so what do I have to do to renew it as I visit Thailand and Laos regularly

What a mess, please state the validity of the visa, when is the 6 months finished? Nevermind your last entry stamp date. By re-entering Thailand just before the visa expires you can get another 60 day stamp and extension in country, getting almost 9 months out of the visa.
On your current visa in total how much time have you spent inside Thailand not counting your current 90 days stamp (which I assume is the final one you can get other than a 30 day extension)
then you should be fine with a 2nd one assuming your home consulate will issue you with one. Unfortunately you would need to travel to your home country to apply
An METV is only available from your home country or country of legal residence.
You can’t renew it but you could go home and buy a new one. Some embassies or consulates may have restrictions on whether you could get more than one in a year.
