Do I need to provide a TM30 form for a visa extension in Thailand on a tourist visa?

TM30 question. I came in on a single entry tourist visa in a British passport. I now need to do the dreaded extension at Chaengwattana. I have not seen a TM30 form and never filled one in. Do I need to do something with regards to this when I go to do the extension? Thanks for any info in advance.
