My non immigrant o visa runs out on 7th Jan, but I have a letter from Phuket mission hospital stating that I cannot travel until 21st Jan due to a recent knee operation.
Can anyone confirm if I should take this letter to Phuket immigration, or do I just take the letter with me when I leave Thailand on the 21st.

As a rule extensions of stays based on medical reasons are not easily handed out. If they dent the extension they'll sell you a 7 day stamp to get out of thailand.
Like people said, go to the immigration office BEFORE your current stamp runs out.
