How can I transfer my visa stamp to a new passport and apply for an extension due to COVID-19?

Hello everyone. I am looking for the "Transfer stamp to new passport form". I checked the files section but could not find it? Just looking to print it off and have everything ready before I head to MTT counter K tomorrow. 

Also do you think I will be able to transfer my current visa stamp (unable to travel due to covid 60 days) into the new passport and apply for my new (under consideration) unable to travel due to covid 60 days stamp all in one go? 

(Australian passport, transferring stamp to new passport,  heading to counter K at MTT for another 60 day covid extension).

Australia embassy puts a Stamp in your old passport saying that a new one issued. No letter required
