I printed out form from internet, attached 4x6 cm picture, printed copies of passport all relevant pages, departure card-they accepted without issues, charged ฿1000. There were reports they require they print it out and take your photo at ฿200 extra, that didn’t happen to me. Also the reentry permit and overstay office is just past where immigration stamps you out of Thailand to the left. There are separate desks and officers for overstays and reentry, it took about 15 minutes including time to help one person in front of me. It wasn’t busy at this moment–1430 on a Thursday afternoon – but I’d allow an hour for the process in case there are several people in front of you.
