Go to the immigration office and get the hand out they have which mentions the documentation you need.

You can apply for an extension when you have 45 days or less left on your current stamp. You can't get more than a year extension.

You'd have to meet the financial requirements of either 400K baht banked in a thai bank account in your name only for 2 months before your application OR a notarized income affidavit from your consulate stating you get 40K baht a month in income OR if you legally work here show 3 months of pay receipts and tax statements that show 40K baht a month in income
