Hi all! We are a UK family of 6 in Chiang Mai. We are staying 12 KMs outside of Chiang Mai city centre.
Our 45 day initial visa exempt period ends on 15th Dec. We intend to extend by another 30 days. Are there forms we can fill in ahead of arrival to the immigration office? If so, how can I get hold of them? Our Air BNB landlord has filled out the TM 30 forms and showed us proof of this so that’s done. If we arrived at the immigration office early (e.g. 12th Dec), would the 30 day extension start from 13th or would they extend from 16th as per our 45 day visa exempt period end date stamp in our passports?

everything on there seems fine except the TM6 departure card. I don’t recall ever seeing one of these when I entered Thailand. Currently we have no definitive plans to leave Thailand (e.g. no onward travel booked) and we actually intend to stay until April by doing a visa border run in mid Jan.
Yes, get the form at the IO as different offices use their own versions of the form. I think at Chiang Mai you can extend when you have 45 days of your entry stamp remaining. Your extension will be added to your current stamp so no harm in going early.
