I need to do the 30 day extension for my current 30 day tourist exempt entry. US passport. Will I need to provide lodging information this time? Do I need to apply in the district I will be staying, or any office is fine?

when you arrived you hadn't already checked in to hotel/ accomodation so wouldn't have a TM30. Arrival Immigration wouldn't ask for this. You're now asking about going to an immigration office to extend stay, different thing to arriving.
you can only file a TM30 for today. You can't file in the past. But that's not an issue.

I was fined 1600b this past Monday because I didn’t notify immigration within three days of my border crossing entry. I couldn’t apply for the 1900b extension without first paying the fine.
Wow. What office was this? Any fine in theory should be paid for by your house owner/ landlord/ hotel, etc who didn’t file. Except that they generally aren’t and you have to stump up the fine if you want service.
Ah ok. That explains it. For future reference they only require a filed TM30. If you stay a night in a hotel and get a print of their filing then you’re good to go. Regardless that you actually arrived in Thailand 29 days ago and didn’t actually have one from any previous stays.
sorry but I don’t understand the date you mentioned, nor bounced your limit. I suspect the latter is referring to you’ve done two land border crossings this year using exempt entries.
