You NEED to go to the immigration office which serves to the address you’d provide. You cannot pick any.

And for sure about requirements, you better to check with that immigration office.

Basically, you need these along your original passport

-copy of

your passport ID page, your entry stamp page, TM6 (departure card)

-passport size photo

-1,900thb fee

Some offices requires copy of TM30 receipt, some offices requires proof of address instead, some don’t.

Some offices may require some more passport page copies.
