When adding extra 30 days to a tourist visa do you need to provide proof of accommodation for the full 30 days , 

Also we will be staying in Phuket can we extend our visas in Krabi as we will be in Krabi for a week? Does the proof of accommodation have to be from the date we go to extend or from the date our tourist visa runs out

You don't need to provide any proof of future accomodations for a tourist extension.

You would apply at the immigration office that correlates with the most recent TM30 filed. So if you check in to a hotel in Krabi that hotel should file a TM30 and you can ask for a copy to file for an extension in Krabi.

Both Phuket nor Krabi require a filed TM30 for a 30 day extension. You just need to provide an address for that province that you are staying in.
