Sorry, stupid question but I just can't remember: For the 30day extension on SETV, do I need some proof of where I'm staying (hotel receipt, etc.) or do I just input the address on the form?

TLDR : Answer Summary
For a 30-day extension on a Single Entry Tourist Visa (SETV) in Thailand, you typically only need to provide your hotel address on the extension application form. This is sufficient according to community members, so a hotel receipt is not required.

How about stating what immigration office you're going to use to get the extension? That's going to affect the answers

Just the hotel address is sufficient.
