Yes at some Immigration offices NO at other offices, best is to find an Immigration Office in your area and ask what they like to see.
Goes much quicker then me typing the requirements of over 70 offices nation wide.
Next to that the Immigration Office can directly check your visa status and tell you if you can apply for a 30 day Extension of Stay or not.
Not every visa can be extended with 30 days.
