No you just need to show proof of addresz, I just recently went for my 2nd 60day extension and they asked my fiance for his ID card to verify

...you really dont need anything.....just show your passport....and that's it ....in and out 10 minutes ....

I wrote my condo address. No proof was required

They ask for a place of residence on the application form. I just booked a hotel in Bangkok for a week and gave them that address. My visa got approval straight away.
