If you get an extra 30 days extension on a visa exemption does the 30 days start from when you apply or does it start from when your last day was stamped in the passport?

TLDR : Answer Summary
The 30-day extension on a visa exemption in Thailand starts from the expiration date stamped in your passport, not from the date of application.

they add extensions on to the expiration date of your current stamp.

There is no reason to wait until it's almost expired.
