I have pensioners VISA with a multi entry VISA, do I still have to get a permit from Immigration to leave Thailand for 14 days?

You can go and come back into Thailand as you please before 21 May 2021. You have an Extension of Stay and a Multiple Re-Entry permit both valid till that date and on every entry you get Admitted until 21 May 2021. Safe travels.
